Free and confidential counseling is available. Coordinator, Daisy de Jesus, MSW, will visit each borough on the following dates.
BROOKLYN First Monday of each month, 3 p.m. to 6 p.m.
QUEENS Second Tuesday of each month, 3 p.m. to 6 p.m.
BRONX Third Wednesday of each month, 3 p.m. to 6 p.m.
STATEN ISLAND Fourth Thursday of each month, 3 p.m. to 6 p.m.

Blood-borne Pathogen (BBP) Exposure Procedure
What to do if you are exposed to Blood or Bodily Fluids
- Contact Site Administrator and report the incident.
- Complete and sign Exposure Incident Report – REV 9504 (which should be faxed to both the regional office and DOE Occupational Safety and Health Dept. within 24 hrs. of incident).
- GET MEDICAL EVALUATION IMMEDIATELY – and ask for a Source Individual Report Form from your site administrator.
