Work Shouldn't Hurt

If you have a work-related injury, illness or accident:
- Seek medical attention immediately
- Report the injury, accident or illness to the principal
- Fill out the necessary forms
- Inform your chapter leader and contact your union:

If you’re the victim of an Assault or Incident call UFT Victim Support 212-598-6853
Check list of forms that must be completed:
- Comprehensive Injury Report
- WCD23-Employee’s Notice of Injury
- Workers’ Comp C-2 Employers Report
- Workers’ Com C-3 Employees Claim
- Workers’ Comp C-4 Physician’s Report
- UFT School Safety Incident Report
- OP 198-Excused Absence with Pay
