What happens if I lost my Teacher's Choice debit card last year and still have money to spend?
If you have lost your debit card, you should call the Chase Customer Service number at 866-795-3890 to request a replacement card. Once the replacement card is mailed to your home address, you should immediately activate the card by calling the Chase Customer Service number provided.
Please refer to the Teacher's Choice Direct Payment Card Holders document for more detailed information regarding the use and activation of your Fiscal Year 2011 debit card, if you have not already used your previous Teacher's Choice funding for last year.
