Enroll for Health Benefits

The union provides an array of benefits to supplement the health plan coverage that you and your eligible dependents receive from the city — but only if you enroll in the UFT Welfare Fund. Remember that this enrollment is separate from your New York City health plan enrollment, which provides your major medical coverage.

You can also complete and submit a hard-copy UFT Welfare Fund enrollment form along with any necessary documentation for your dependents. You can get an enrollment form at your school from your UFT chapter leader or by calling the Fund forms hotline at 212-539-0539.
