Open enrollment ends Oct. 15

The annual open enrollment period for the UFT Welfare Fund dental plans is from Sept. 1 through Oct. 15. Members who wish to change their dental plans must submit a dental enrollment/transfer form to the UFT Welfare Fund. Any newly selected plan will become effective Nov. 1.
If you do not wish to change your dental plan, there is nothing that you need to do. Your dental coverage will remain the same.
To make a transfer, call the UFT Welfare Fund Forms Hotline at 1-212-539-0539 for a dental enrollment/transfer form or you can download the form here. Contact the member services department at 1-212-539-0500 for more information concerning your dental plan options.
