There's a wealth of information on the Welfare Fund web site

Facts, details, forms and links are among the many features available to members on the ever-expanding Welfare Fund Web site, www.uftwf.org. Members can, among other things, read about their benefits in the Red Apple, find details about the drug program, look for participating optical or hearing aid centers and obtain downloadable forms for almost all the many benefits the Fund offers.

There are also links to the major health plan Web sites, Summary Program Descriptions (SPDs), SIDS dental panelists and the drug program administrator.

However, members should be aware that the site does not include forms for optical or hearing aid certificates. These forms are not included because they must be validated before being issued.

Members who are not Web-connected may obtain any Welfare Fund form from their chapter leader or from the Fund’s hotline, 1-212-539-0539.
