The easy way to enroll or update your information. New members: You must enroll in the Welfare Fund after you join the UFT to enjoy dental, optical, prescription drug and other benefits. This enrollment is separate from your enrollment in a New York City health plan.
The quickest way by far is to enroll online: On the Welfare Fund Web site.
You can also change your status online.
In addition, the Welfare Fund allows an online process to enable members to update their information. Members may change their address or dependent information by using the online process.
But for the technologically challenged, Welfare Fund enrollment forms and blue Change of Status forms are also available from chapter leaders or by calling the Fund’s Hotline: 1-212-539-0539.
Supporting documentation must be sent to the Fund following either process.
