Welfare Fund September open enrollment for dependent children

Age 26 coverage

  • Dependent children who were certified with the UFT Welfare Fund as full-time students as of May 2010 have automatically had their coverage extended to age 26. There is nothing parents need to do at this time.
  • Children under age 26 who were not certified as full-time students in May 2010 will be able to enroll their child with the Fund during the month of September 2010. During this “special open-enrollment” parents must complete the Application for Age 26 Young Adult Coverage for Welfare Benefits Only form. Once the completed application and required documentation is received, your dependent child will be eligible for UFT Welfare Fund benefits, also at no cost, effective Oct. 1, 2010.

If your dependent child has never been covered by the UFT Welfare Fund in the past, please complete the online Update Your Information form which will add your dependent to your record.

Note: UFT Welfare Fund coverage for your dependent child is separate from New York City health plan coverage, which will become available in July 2011. For information and to enroll your dependent child in the New York City Health Benefits Program, visit the program’s website at www.nyc.gov/olr.

Please note: you should consider purchasing the City health plan coverage for your dependent child because the health plan provides medical, hospital and programs such as the New York City PICA program which provides injectable and chemotherapy drugs that are not covered by the Welfare Fund.

Age 29 coverage

  • If you wish to purchase the age 29 coverage (for members whose children are between ages 26 – 29), members need to complete the Application for Age 29 Young Adult Coverage UFT Welfare Benefits Only form. If your child has never been covered by the UFT Welfare Fund in the past, you will need to complete the Update Your Information form with accompanying documentation and a separate application.

Eligibility begins the first of the month following receipt of the application and payment. Coverage terminates on the 30th birthday.

Note: UFT Welfare Fund coverage for your dependent child is separate from New York City health plan coverage, which will become available for purchase in July 2011.
