Healthcare rebate application
The United Federation of Teachers is proud to announce the return of its Healthcare Rebate Program to address the unique needs of Hearing Officers Per Session and their families.
The program is offered exclusively to members of your chapter. It provides a lump sum payment to offset the costs of healthcare expenses for yourself and your immediate family members. These expenses include, but are not limited to, insurance premiums, copays for doctor visits, out-of-pocket expenses for medical care, medical equipment, eyeglasses, prescriptions, vision and dental care.
Please note, this benefit is available only to UFT members. To qualify for the 2023 payment, you must have worked at least 400 hours at Office of Administrative Trials and Hearings during the calendar years of 2021 and 2022 combined.
Below, you will find links to the materials necessary to apply. There are two simple forms to complete and submit.

The application deadline is Sept. 30.
The application instructions are as follows:
- Complete the forms, attach documentation of your expenses from calendar years 2021 and 2022 for up to $3,000 and submit these materials — signed application, expense worksheet(s) and documentation of expenses — in one packet.
