APPR complaint
This online form will be suspended over the summer break as of 12:00 a.m. on June 27 but will be reactivated on Tuesday, Sept. 8. For issues related to summer school, please call the union at 212-331-6311 or contact your UFT borough office.
Teachers covered by the Advance evaluation system may use the APPR complaint form to ask their principal for assistance in resolving an issue with the implementation of the teacher evaluation system. The teacher must file this form within five school days of knowledge of the act or occurrence that led to this request.
