APPR complaint

Use this complaint form to ask your principal for assistance in resolving an issue with the implementation of the teacher evaluation system. You must file this form within five school days of knowledge of the act or occurrence that led to this request.

Please note that you must be logged in to in order to access this form. This form is only for use by teachers covered by the Advance evaluation system.

NOTE: Effective March 18, due to the COVID-19 pandemic, the online APPR Complaint process is suspended. You are encouraged to attempt informal resolutions in the meantime. If you have any questions, please contact the UFT at 212-331-6311.
